The Health Information Management Department (HIMD) is located at the 2nd floor of Valenzuela Medical Center at Window 1. It is available from Monday to Friday, 8 am to 5 pm.
Reminders:
You can only request through this application once the patient has already been discharged (for inpatients) or done consultation (for OPD/ER patients). Request of patients who are still admitted will automatically be denied.
Please wear facemask and faceshield. Observe physical distancing within the hospital premises. Bring the neccessary sss forms and pertinent documents for your request to the HIMD window.
Please bring the necessary SSS forms and other documents upon request of notification claims/ insurances on HIMD window.
Proceed to HIMD window only when your queueing number is called.
